Policies & Procedures

At SNF NYC, our goal is to help individuals and families navigate the process of identifying nursing homes, skilled nursing facilities, short-term rehabilitation centers, and long-term care options throughout New York City. We are committed to providing professional, respectful, transparent, and responsible assistance while protecting the privacy and dignity of every individual and family who contacts us.

Our Role

SNF NYC is a healthcare placement and referral resource designed to assist families in identifying care facilities that may meet their loved one’s needs. We help families understand available options and connect them with appropriate facilities based on factors such as location, level of care, rehabilitation needs, specialized services, insurance considerations, availability, and family preferences.

SNF NYC does not operate nursing homes, rehabilitation centers, hospitals, or other healthcare facilities and does not provide medical, nursing, rehabilitation, legal, financial, or insurance advice. Final admission decisions, clinical assessments, insurance verification, eligibility determinations, treatment plans, and care decisions are made by the applicable healthcare providers, facilities, insurers, patients, and authorized representatives.

No Cost to Families

There is no cost to patients or families for SNF NYC placement assistance. Families may contact us to discuss their needs, explore potential care options, and receive assistance connecting with appropriate facilities without paying SNF NYC a placement or consultation fee.

SNF NYC may maintain professional referral or marketing relationships with certain healthcare organizations or facilities. When applicable, SNF NYC may receive compensation from participating organizations. Such compensation does not change the amount charged to the patient or family by SNF NYC for our placement assistance.

Referral and Placement Process

Our process begins with understanding the individual’s situation and care needs. This may include the desired location, type of rehabilitation or nursing care required, anticipated length of stay, insurance coverage, specialized services, accessibility requirements, and other preferences communicated by the patient or family.

Based on the information provided, SNF NYC may identify facilities that appear potentially appropriate. Whenever possible, families are encouraged to speak directly with facilities, review available information, ask questions, tour facilities, and make their own informed decision before selecting a provider.

A referral or recommendation from SNF NYC does not constitute a guarantee of admission, insurance coverage, availability, quality of care, clinical outcomes, or suitability for a particular patient.

Facility Selection

SNF NYC strives to provide families with useful options based on the information available to us at the time of the request. Facility availability, services, staffing, insurance participation, admission criteria, ratings, ownership, programs, and other conditions may change.

Families should independently verify information directly with the facility and appropriate healthcare or insurance professionals before making a final decision. We encourage families to consider factors such as licensing information, inspection history, staffing, specialized programs, location, visitation policies, insurance participation, and the individual needs of their loved one.

Privacy and Confidentiality

SNF NYC respects the privacy of individuals and families who contact us. Personal information provided to SNF NYC will be handled with care and used for legitimate purposes related to responding to inquiries, assisting with placement or referral requests, communicating with families, and coordinating with appropriate organizations when authorized or otherwise legally permitted.

We seek to collect only information reasonably necessary to provide requested assistance. Individuals should avoid submitting unnecessary sensitive medical or financial information through unsecured website forms, text messages, or email.

Where protected health information is involved, information will be handled in accordance with applicable privacy requirements and any contractual obligations that apply to SNF NYC.

Consent to Communicate

By contacting SNF NYC and providing contact information, individuals authorize our team to respond to their inquiry using the contact methods they provide, subject to applicable law and their stated communication preferences.

Where permission is provided, SNF NYC may communicate with family members, authorized representatives, healthcare professionals, discharge planners, social workers, case managers, and potential care facilities for the purpose of assisting with a placement or referral.

Individuals may ask SNF NYC to stop nonessential communications at any time.

Accuracy of Information

SNF NYC makes reasonable efforts to provide helpful and current information; however, we cannot guarantee that all information received from facilities, third parties, online resources, patients, or family members is complete, accurate, or current.

Families are responsible for confirming important information directly with the facility, including availability, admission requirements, insurance participation, services offered, costs, room arrangements, visiting policies, and other conditions before admission.

Insurance and Financial Matters

SNF NYC does not guarantee Medicare, Medicaid, managed care, private insurance, or other coverage for any facility or service. Insurance participation and benefits can vary by facility, plan, patient eligibility, authorization requirements, and medical necessity.

Patients and families should verify coverage, deductibles, copayments, coinsurance, private-pay charges, authorization requirements, and other financial responsibilities directly with their insurance carrier and the selected facility.

Medical Emergencies

SNF NYC is not an emergency medical service. Our website, telephone number, forms, and referral services should not be used for medical emergencies.

If someone is experiencing a medical emergency or potentially life-threatening condition, call 911 or seek immediate assistance from an appropriate emergency medical provider.

Equal Access and Non-Discrimination

SNF NYC is committed to treating individuals and families with dignity and respect. We seek to provide our services without unlawful discrimination based on race, color, national origin, age, disability, sex, religion, or any other status protected by applicable law.

Facility admission decisions are independently made by the individual facilities according to their admission criteria, capabilities, availability, and applicable laws and regulations.

Complaints and Concerns

We take concerns regarding our services seriously. If a patient, family member, healthcare professional, or facility has a concern about an interaction with SNF NYC, we encourage them to contact us directly. We will review concerns in a professional and timely manner and take reasonable corrective action when appropriate.

Concerns regarding the quality of care provided by an independent nursing home, rehabilitation center, or healthcare provider should be directed to that organization and, when appropriate, the applicable regulatory authority.

Professional Conduct

SNF NYC representatives are expected to communicate honestly, professionally, respectfully, and compassionately. Our team should never pressure a family into selecting a particular facility. Our purpose is to provide information, facilitate connections, and help families navigate available options while respecting the patient’s and family’s right to make their own decisions.

Conflicts of Interest and Transparency

SNF NYC recognizes the importance of transparency in healthcare referrals. Where SNF NYC has a financial or business relationship with a participating facility, we seek to conduct referrals responsibly and in accordance with applicable laws and contractual requirements.

Families remain free to select any facility they believe is appropriate and are not required to choose a facility that has a relationship with SNF NYC.

Updates to Our Policies

SNF NYC may periodically update these Policies & Procedures to reflect changes in our services, business practices, technology, or applicable requirements. Updated policies may be posted on our website with a revised effective date.
