Finding the Right Care

When someone you love needs short-term rehabilitation, skilled nursing care, or long-term care, families are often required to make important decisions very quickly. A hospital may be preparing your loved one for discharge, a physician may recommend rehabilitation, or changing health and daily living needs may make it difficult for your loved one to remain safely at home. Suddenly, your family is faced with questions about nursing facilities, rehabilitation programs, insurance coverage, location, specialized services, availability, and quality of care.

At SNF NYC, we help make this process easier to understand and navigate. With over 20 years of experience in healthcare and rehabilitation, we help families explore appropriate care options based on their loved one's individual situation. From the first conversation through connecting with potential facilities, our goal is to provide guidance, information, and support during an often stressful transition.

Our services are provided at no cost to families. Families DO NOT pay SNF NYC a consultation, referral, or placement fee for our assistance.

Step 1 — Tell Us About Your Loved One

The process begins with a conversation. We take the time to understand what is happening with your loved one and what your family is looking for.

Every situation is different. Someone recovering from a hip replacement may require intensive physical and occupational therapy before returning home. Another person may need skilled nursing following a hospitalization, while someone else may require ongoing long-term care and assistance with everyday activities.

We discuss important factors such as the type of care being recommended, rehabilitation needs, preferred geographic area, insurance considerations, specialized services that may be required, and the family's preferences.

Our goal is to understand the whole situation—not simply provide families with a list of nursing homes.

Step 2 — Determine the Appropriate Level of Care

Understanding the difference between care options can be confusing, especially when a family is dealing with these decisions for the first time.

Depending on the individual's needs and professional care recommendations, we can help families explore options involving short-term rehabilitation, skilled nursing care, and long-term care.

Short-term rehabilitation is commonly needed following hospitalization, surgery, illness, injury, or another medical event. Rehabilitation may include physical therapy, occupational therapy, speech therapy, nursing care, and other services intended to support recovery and greater independence.

Long-term care may be appropriate when an individual requires ongoing nursing supervision and assistance with daily activities and can no longer safely receive the necessary level of care at home.

We help families better understand these options so they can have more informed conversations with their loved one's healthcare providers and potential facilities.

Step 3 — Identify Potential Facilities

Once we understand your loved one's needs and your family's preferences, we help identify nursing and rehabilitation facilities that may be appropriate for the situation.

Many factors can influence the search. Location may be extremely important because families want to remain close enough to visit regularly. Other considerations may include rehabilitation programs, specialized nursing services, accessibility, language or cultural preferences, facility environment, insurance participation, and the individual's particular care needs.

Instead of calling facility after facility without knowing where to begin, families can receive assistance narrowing the search to potential options that better match their circumstances.

Step 4 — Help You Compare Your Options

Choosing a facility involves much more than finding an available bed.

Families should feel comfortable asking questions and understanding what each facility can provide. We help families consider the factors that may matter most for their loved one's care.

This may include the facility's rehabilitation services, nursing capabilities, specialized programs, location, environment, communication with families, visiting considerations, insurance participation, and other individual priorities.

The final decision always belongs to the patient and family. Our role is to help make the process more manageable and provide information that can help families evaluate their choices.

Step 5 — Connect With the Facility

After potential facilities have been identified, we can help connect the family with the appropriate admissions representatives so they can discuss the patient's circumstances, admission requirements, availability, insurance, and next steps.

Admission decisions are ultimately made by the individual facility based on its admissions criteria, ability to meet the patient's needs, bed availability, insurance or payment arrangements, and other relevant factors.

Our role is to help facilitate communication and make it easier for families to reach the right people.

Step 6 — Coordinate With the Existing Discharge Process

When someone is leaving a hospital, the hospital's discharge planning team, social worker, case manager, physicians, therapists, and other professionals may already be involved in determining the appropriate next level of care.

SNF NYC does not replace these healthcare professionals. Instead, we can serve as an additional resource for the family as they navigate their choices.

Families should continue working closely with their loved one's healthcare providers and hospital discharge team regarding medical recommendations, discharge readiness, transportation, medications, documentation, and other clinical decisions.

Step 7 — Prepare for the Transition

Moving from a hospital or home into a rehabilitation or nursing facility can be an emotional transition for everyone involved.

Families may have questions about what happens next, what personal belongings to bring, who they should contact at the facility, what rehabilitation may look like, and what to expect during the first few days.

We want families to feel more prepared and informed as they move through this transition. Knowing what questions to ask and what to expect can make an unfamiliar process feel considerably more manageable.

There Is No Cost to Families

Families already have enough to think about when someone they love needs nursing or rehabilitation care. Finding help should not create another financial burden.

SNF NYC's assistance is provided at no cost to families. There is no consultation, referral, or placement fee charged to families for using our service.

We believe families should have access to knowledgeable guidance when navigating care options, particularly when decisions must be made quickly.
